También ocurre que al ver la TV por cable, al volver a ver el HTPC (con el cambio de entrada del receptor), el ordenador no detecta la salida de audio hdmi. Para solucionar esto he de reiniciar o cerrar la sesión y volverla a abrir.

Anyway, download MonInfo (Monitor Asset Manager 2.7) from here (download link is on the left). I made sure that my PC was booted up correctly and able to configure multiple speaker configurations in the sound control panel. Started the MonInfo program, select file on the top left and create an INF. This should make an INF of your display + receiver. Take note of where you save it to. Now open up device manager, navigate to monitors, and then select your display device (it's probably named generic display device) and click update driver software. You want to browse my computer for driver software and then let me pick from a list... select the INF you created earlier. Reboot, done.
I tested this out a bit and everything is working the way I want it to so far (desktop icons stay in place, audio options don't drop to 2 channel when switching inputs or if I turn on the TV first by accident, etc etc). This fix should work the same way for NVidia GPUs too - I believe that any change in audio/video drops audio altogether in most cases with Nvidia.

Por si se te resiste algo: en el primer enlace explican todo el tema moninfo (Monitor Asset Manager), pero en tu caso ten en cuenta las instrucciones anteriores, es decir, que tu tienes que capturar el EDID con TV y AVR conectados.

Display Device EDID Capture
Connect the graphic card HDMI to your display device HDMI.
IF using Moninfo THEN
Launch Moninfo
"[Real time ...]" entry on top left "Display IDs" window [Select]
File [Select]
Create INF [Select]
SAVE TO [Your display device name].inf. (e.g. Samsung LN-S4095D.inf)

AVR/AVP EDID Capture
Connect the graphic card HDMI to your AVR/AVP HDMI.
[Optionally] Connect the AVR HDMI port to the display device HDMI-out.
IF using Moninfo THEN
Launch Moninfo
"[Real time ...]" entry on top left "Display IDs" window [Select]
File [Select]
Create INF [Select]
SAVE TO [Your display AVR/AVP name].inf. (e.g. DENON AVR988.inf)

Y un tip por si tienes problemas con la instalación del override en W8:

Finally got it working. For anyone else trying to get one of these overrides going in Windows 8, you must disable signature enforcement before it will allow you to click through ("install anyways") on the inf install.

To do so:
From the Metro Start Screen, open Settings (move your mouse to the bottom-right-corner of the screen and wait for the pop-out bar to appear, then click the Gear icon).
Click ‘More PC Settings’.
Click ‘General’.
Scroll down, and click ‘Restart now’ under ‘Advanced startup’.
Wait a bit.
Click ‘Troubleshoot’.
Click ‘Advanced Options’
Click ‘Windows Startup Settings’
Click Restart.
You'll eventually get the option to disable driver sig check. Install the inf, then reboot normally, the mod should stick.

Hace unos meses le expliqué el tema del moninfo a un compañero del foro (en su caso por problemas con los audios HD) por si quieres usarlo como apoyo (es lo mismo que lo dicho antes, pero en tu caso prestando atención a que tu el EDID lo tiene que capturar con TV y AVR conectados):
